Framing

Population health is not somebody else's job. It is the assessment lens that makes individual care defensible.

Narrative

Assessing the priorities of the community and the affected clinical population is the sub-competency that most directly translates the AACN's population-health domain into bedside-relevant practice. A home dialysis nurse who skips this step ends up teaching the same protocol to a patient living alone in subsidized housing and to a patient living in a multi-generational home with a caregiving spouse, and is surprised when one fails. The skill is not running a population-health study; it is the habit of asking, before any teaching or care plan: what does this specific clinical population need, what does this specific community make hard, and where does the gap between the two sit. We teach it because the role-transition work showed it was the most consistently missing assessment step in the curricula we reviewed.

Evidence

Coverage report from the In-Center Dialysis to Home Dialysis methodology run. Educator interviews on what gets dropped when population-health is taught in isolation from clinical practice.

Anchors note

Behavioral anchors are still in authoring for 3.1c. The Progression Indicators below use the AACN level descriptors directly.

Pre-selected as a hero sub-competency: 3.1c is the cleanest example of an AACN sub-comp that academic curricula teach and clinical orientation skips.

Progression Indicators

The three-bucket rubric for this sub-competency. Behavioral anchors are shown when available.

Not Demonstrated

No evidence yet. Either the behavior has not been performed in a captured context, or the captured evidence does not yet meet the rubric.

Developing

Matches the AACN Developing descriptor for this sub-competency. Caps at Developing in solo activation mode for P3 (Hands) sub-comps without preceptor observation.

Developed

Matches the AACN Developed descriptor. For P3 sub-comps this requires a preceptor or peer observation to triangulate the self-report.

No anchored novice, advanced beginner, or competent text on file for 3.1c yet. The AACN level descriptors are used as the rubric source. Anchor authoring is ongoing.
