Framing
Patient and family education is not a discharge handout. It is the single behavior most strongly correlated with whether someone returns to the hospital within thirty days.
Narrative
Educating individuals and families for self-care is the keystone behavior of a nurse working in any transition: discharge from acute care, the move from inpatient to outpatient services, the post-partum hand-off to a new mother. The work splits into three sub-tasks that are easy to conflate: assessing what the patient and family already understand, choosing a teaching modality that matches their cognition and stress, and verifying retention against the specific demands of life after discharge. The AACN sub-competency captures all three. Most curricula teach the first and the third and skip the middle. We teach all three because the failures cluster in the middle, when a nurse demonstrates a behavior the patient cannot replicate alone at 2am.
Evidence
Voice-memo analysis with home dialysis preceptors. Reflective writing from BSN students post-clinical. Coverage gaps in three residency curricula reviewed under the Kaira methodology pipeline.
Anchors note
Behavioral anchors are still in authoring for 2.8c. The Progression Indicators below are the rubric the AACN itself uses for this sub-competency; the anchored novice / advanced beginner / competent text will land in the next framework version.
Pre-selected as a hero sub-competency: AACN 2.8c is the centerpiece of the In-Center Dialysis to Home Dialysis role-transition lead magnet and the most frequently cited sub-competency in clinical-instructor interviews.
Progression Indicators
The three-bucket rubric for this sub-competency. Behavioral anchors are shown when available.
No evidence yet. Either the behavior has not been performed in a captured context, or the captured evidence does not yet meet the rubric.
Matches the AACN Developing descriptor for this sub-competency. Caps at Developing in solo activation mode for P3 (Hands) sub-comps without preceptor observation.
Matches the AACN Developed descriptor. For P3 sub-comps this requires a preceptor or peer observation to triangulate the self-report.
No anchored novice, advanced beginner, or competent text on file for 2.8c yet. The AACN level descriptors are used as the rubric source. Anchor authoring is ongoing.
