"How do you bring it together?"

Framing

Self-determination is where nursing's professional identity diverges most clearly from medicine's. Respect for it is not a soft skill.

Narrative

When a patient declines a recommended treatment, the nursing behavior that follows is not persuasion. It is documentation of informed refusal, escalation to the appropriate decision-maker, and adjustment of the plan of care. The sub-competency captures both a stance (the patient's framework for their own life takes precedence) and a discipline (the stance has to be operationalized in a chart note, a family meeting, and a discharge plan, not just held privately). We pair this sub-competency with 2.8c because the two are often in tension: a nurse can give excellent education and still be required to respect a decision they consider clinically suboptimal.

Evidence

CNO and Director-of-Nursing-Education interviews on what new graduates struggle with most in year one. Reflective writing from ABSN students after first solo family conferences.

Anchors note

Behavioral anchors are still in authoring for 2.8d. The Progression Indicators below use the AACN level descriptors directly.

Pre-selected as a hero sub-competency: 2.8d is the relational counterweight to 2.8c and the sub-comp Dr. Patel cited as the hardest to assess in classroom-only formats.

Not Demonstrated

No evidence yet. Either the behavior has not been performed in a captured context, or the captured evidence does not yet meet the rubric.

Novice anchor: Defers to medical plan even when patient expresses different preferences; uncomfortable raising patient's wishes with team.
Developing

Matches the AACN Developing descriptor for this sub-competency. Caps at Developing in solo activation mode for P3 (Hands) sub-comps without preceptor observation.

Advanced beginner anchor: Respects patient and family self-determination; raises patient's wishes with team when plan and preferences diverge; documents preferences.
Developed

Matches the AACN Developed descriptor. For P3 sub-comps this requires a preceptor or peer observation to triangulate the self-report.

Competent anchor: Advocates for patient self-determination including end-of-life decisions; navigates conflicts between family and patient preferences.
